Enrolments
Enrolment at Ashdale Primary School is a two step process.
STEP 1 - Application for Enrolment
Complete an Application for Enrolment in a Public School, and return to the school with the documents listed below:
Application for Enrolment Form
It is a requirement of the school, that we sight and photocopy the originals of the following documents:
- Birth Certificate.
- Australian Childhood Immunistion Register (ACIR) Statement provided by Medicare (available from the Medicare website).
- Proof of Residency, two documents including any of the following; Current Shire Rates notice, Utilities Account, Rental Agreement (current). Mail addressed to PO Boxes will not be accepted.
- Visa documentation or Citizenship certificates.
- Most recent School Report and NAPLAN results.
Email and attach above documents to ashdale.ps.enrolments@education.wa.edu.au. When attaching documents please ensure they are in pdf format and not screenshots/photos.
You will be contacted by a School Representative to confirm the next steps.
STEP 2 - Enrolment Forms
Once the school has approved your Application for Enrolment form, you will be directed to complete the following documentation for the appropriate year level you are applying for.
If you are applying for Kindy, we will contact you in August/September to complete the next stage of the enrolment process.
